Create and Maintain Surveys

Please read topic About Surveys before creating or editing your first survey. A survey can be attached to two types of training items - WBT and ILT. The minimum permission needed to perform this task is Division Author.

Note: One survey can be attached to many training items, but each training item can only have one survey.

How to Create a Survey

Follow the steps provided below to create a survey.

Select Author > Survey > (Organization level, if shown) from the system menu.

If you have access to multiple divisions or companies, select the division or company where you want to create the survey.

Result: A blank form is displayed with the survey section on the left side and the training item section on the right side.

Enter the Survey Title and Survey Code.

Note: You can use the same title and code for both survey and training item.

Enter values in Highlight results. The Survey Details Report will highlight all the results within this range, allowing you to pinpoint results in the specified point range. Enter zero in both fields if highlight is not needed.

Enter Objectives and Notes.

Note: These fields are optional, but are seen by the learners when they launch and complete the survey.

If you are at company level and creating a survey for a division, select the Division from the drop down list.

Does the training item for this survey already exist?

If Yes, click Existing, enter the search criteria, using % as a wildcard, if necessary. Click Search and select the training item from the list.

If No, enter the Training Item TitleItem CodeItem Type, and Hours Credit.

Note: You can use the same title and code for both survey and training item.

Result: The survey form is displayed.

Add Questions and Answers to the survey.

How to Add Questions and Answers

Follow these steps to add questions and answers to a survey.

Select the survey to which you want to add questions and answers.

Do you want to Import questions from another survey? This is not very common.

If YES, follow the procedure Import Survey Questions.

If NO, click Question: Add on the action toolbar.

Result: Step 1 of the Add Question Wizard opens.

Select the question type.

Result: Step 2 of the Add Question Cell Wizard opens.

How to Add Question and Answer Content

Follow these steps to add questions and answers content to a survey.

Type in the question text here. Your question will automatically be formatted in the default professionally designed style. This will be applicable in a majority of situations.

enter an image or add formatting to the question, such as special font or font color

Click paste HTML and enter the question and the desired image in the HTML editor. Use the toolbars to format the text and insert images. You must click paste to transfer the information entered to Create Cell wizard.

Select Next.

Result: Step 3 of the Add Question Cell Wizard opens. It is customized for the question type you selected.

Enter the Answers.

Note: If the answer text is long, double-click on the answer text field to display a box that lets you edit the entire answer.

If you need more than four answers, click Add Answers.

Add Points for each answer.

Select Finish.

Select the Preview tab to see the question as it will be displayed to learners. The Content tab shows points for each answer. The number of points is not displayed to learners.

If you want to edit the question, answers or points, click the Content tab. Remember to click Update if you make any changes.

Do you want to add another question?

If Yes, go to Part 2 - Add Questions and Answers - step 3.

If No, close the survey layout window by clicking the X on the top right of the window.

Result: You are returned to the survey form. Total No. of Questions and No. of Questions to Ask are automatically filled in with the number of questions you entered.

Select Preview on the survey form and view the survey.

How to Import Survey Questions

Follow these steps to import questions with answers from another survey.

Select General: Import Survey Questions.

Result: The Import survey question wizard is displayed.

Enter the first few letters of the survey from which you want to import questions in the Survey Title box and click .

Result: A list of surveys matching the search criteria is displayed.

Select the survey from the list and click OK.

Select Display Questions.

Check the box(es) next to the questions you wish to import.

Select Import.

Result: All the survey questions that you checked are imported.

How to Attach a Survey to a Training Item

A survey can be attached to more than one training item. Follow these steps to attach a survey to a training item.

Result: A list of training items to which the survey is currently attached is displayed.
