About Surveys
You can create surveys of any size. Surveys can be designed with up to 5 different types of questions. A survey must be attached to a training item so that it can be delivered to learners. Using survey reports, you can view both detailed and aggregate survey results.
Survey Details
You can list the existing surveys, create or edit surveys from Author > Surveys. Clicking on a survey name or the form view shows the survey details form.
The minimum system code needed to create or edit surveys is Division Author. This table describes the key fields on the survey details form.

Displays the total number of points generated by the learner at the end of the survey. |
Highlight results between |
The survey details report will highlight all the results within this range, allowing you to pinpoint results in the specified point range. If highlight is not needed, enter 0 in both fields. |
Notes |
Notes are displayed to learners at the end of the survey. A typical note is: "Thank you for participating in the survey." |
Training Items
The Training Items tab shows the training items to which the survey is attached. Surveys can only be attached to training item types WBT and ILT. You can attach the same survey to more than one training item. For example, if you create a generic survey for classroom training feedback, it can be attached to many ILT training items. However, a training item can only have one survey attached.
Records
The Records tab shows all the learners who have completed the selected survey. The score (total number of points for the learner) are shown in both the list and the form view; allowing you to quickly find learners in specific point range.
Preview
The Preview button allows you to see the survey as it will be displayed to learners.
Survey Layout
The Layout button gives you the complete layout for the selected survey.
This functions similar to Lesson > Layout, except that all the cells are question cells. The Survey Layout form is divided into 3 sections:
Action Toolbar - at the top of the screen.
Survey Structure - on the left side of the screen.
Cell Details - on the right side of the screen.
Survey Layout Action Toolbar
The Action toolbar is divided into 2 parts: Question and General.
Question:
- Add launches the Create Cell wizard.
- Cut deletes the cell, but saves a copy in memory.
- Copy copies the cell.
- Paste pastes the cell below the selected cell.
- Delete deletes the cell.
General:
- Print Survey prints the entire survey as seen by the learner.
- Import Survey Questions allows you to import one or more questions from an existing survey. The questions are added to the end of the survey.
Survey Structure:
Survey question cells are listed on the left side of the Survey Layout screen.
Cell Details:
The cell details section has 3 tabs: Properties, Content and Preview.
The Properties tab contains these fields:
- Cell Type is automatically filled in as Question Cell.
- Cell Title is automatically generated when you insert a new cell.
- Order is automatically generated.
- Question Type can be one the following:
- All that Apply: One or more answers can be selected.
- Multiple Choice with Essay: Only one answer choice can be selected. Space is provided to explain the answer.
- Multiple Choice: Only one of up to eight possible answers can be selected.
- True False: Either true or false can be selected.
- Yes No: Either yes or no can be selected.
The Content tab allows you to add or edit cell content.
The Preview tab allows you to see the survey questions and answers, with the number of points for each answer.
Delivering a Survey to Learners
A survey must be attached to at least one training item. Surveys can be attached to WBT and ILT type training items only.
When both a survey and a test are attached to a WBT training item, and Allow Test-Out is not checked in the training item form, the learner must complete the survey before taking the test. Therefore, the learner is forced to complete the survey before getting credit for the training item. If you create a WBT type training item specifically to deliver a survey, you must still create a lesson and attach it to the training item. The lesson may contain only one page, containing the title and purpose of survey.
A survey can also be attached to an ILT (instructor-led) training item to gage the effectiveness of classroom training. One generic survey can be attached to many ILT training items. If a survey is attached to an ILT training item, a learner cannot record attendance unless he or she completes the survey first.
Survey reports
These reports are available from the Reports > Survey menu:
- Class Survey Statistics - displays the percentage of people who chose each answer on a survey for a specific class. If the question is Multiple Choice with Essay, the report will display each learner's essay answer. You must select the class for which you want to run the survey statistics.
- Survey Answers - lists all learners' answers to a particular survey question.
- Survey Details - lets you views surveys taken by a particular person. You can further select a particular survey and look at the details. The survey results which fall in the range selected on the survey form are highlighted in yellow.
- Survey History - lets you create survey records reports for your employees. You can create one or more customized reports using score as one of the fields to categorize your risk areas.
- Survey Statistics - displays the number and percentage of people who chose each answer in the survey, allowing you to prioritize problem areas identified by the survey.
